A 73-year-old man with Type 2 diabetes and lymphoedema, under the care of the community within the UK, with a diabetic foot ulcer. On presentation to the wound clinic had a non-healing wound on the right hallux (big toe), of 18-month duration.

  • Initial Assessment

    Pain score VAS 5

Initial assessment found the wound to be 2cm in length and 3.5 cm in width low to moderate exudate. There was 25% sloughy, 50% granulating and 25% epithelialising tissue present. The peri-wound had signs of maceration due to fluctuations in exudate. The wound had stalled and showing signs of infection. ActivHeal® PHMB Foam Silicone Border was selected to assist in reducing wound bioburden, manage exudate and maintain a moist wound environment.

  • Week One

    Pain score VAS 2

The wound at week 2 showed good progress, with the wound reducing in size to1.9cm length, 2.5cm width. The wound was now showing 90% granulating and 10% epithelising tissue. The peri wound continued to show signs of maceration due changes in exudate level from the patients ongoing lymphoedema. The bioburden and signs of infection were decreasing, ActivHeal® PHMB Foam Silicone Border was continued

  • Week Two

    Pain score VAS 0

At week 3 the wound had continued to reduce in size now 2.2cm length and 1.5cm width. The wound had 100% granulating tissue. Both the patient and clinician were impressed with the progression of the wound, and there was no evidence of infection. ActivHeal® PHMB Foam Silicone Border was discontinued, and non-antimicrobial dressings were used.
